Medical Social Worker - HIV Medical Case Management
Clinical Psychiatric Nurse Practitioner
20-hour a week, part-time position
The Clinical Psychiatric Nurse Practitioner is responsible for diagnosing, treating and prescribing appropriate medications for Behavioral Health patients. This position will also be responsible for completing appropriate chart documentation per departmental policy and collaborate with the Behavioral Health treatment team to improve patient treatment. Data abstraction will occur internally and at external partner locations.
Duties and Responsibilities:
• Provides care for client caseload including assessment, diagnosis and medication monitoring in cooperation with therapists and counselors
• Prescribes clinically appropriate psychiatric medication
• Works with case managers to sign off on Patient Assistance medication applications
• Completes all required chart documents
• Ensures compliance with all HIPAA regulations
• Works in collaboration with all clinic providers to ensure holistic approach to client treatment
• Assists in writing project reports, presentations, and articles for presentations
Required Knowledge, Skills and Experience:
• Psychiatric Nurse Practitioner with licensure to practice in Missouri
• 3-5 years of experience as a Psychiatric Nurse and/or Nurse Practitioner
• Knowledge of quality management/continuous quality improvement processes
• Clinical knowledge of psychiatric nursing with emphasis on SPMI populations
• Attention to detail
• Ability to work as a tem member
• Excellent verbal and written communication skills
• Ability to relate to a broad and diverse spectrum of our community
• Experience with infectious diseases preferred
Medical Social Worker – HIV Medical Case Management
Are you an innovative Social Worker, experienced in health care and Strengths Based case management? Lots of people can manage cases. We’re looking for someone with heart that is a seasoned BSW or MSW (minimum qualification) with 2+ years experience in mental health or medical case management.
Interested in meeting new people? A group of diverse clients ages 18 and above want to get to know you! This position provides Strengths Based Case Management to adults living with HIV/AIDS in order to support their access and engagement in medical care.
